MCPN's service area includes Arapahoe, Jefferson, Adams, and Park Counties and the cities of Lakewood and Aurora. Services within the area are developed to provide primary healthcare for people who have no other access to healthcare. A Family Practice model of healthcare is supported by case management services, pharmacy services, and other coordination efforts to remove barriers to healthcare. Well Child Care, immunizations, obstetrics, gynecology, and chronic disease management are among the most often-requested services. Staff, fluent in languages other than English, help remove cultural barriers to care. To become a patient you must first contact the MCPN Customer Care Call Center.
